Donepezil HCl
Indications/Uses
Symptomatic treatment of mild, moderate & severe Alzheimer's disease. Vascular dementia (dementia associated w/ cerebrovascular disease). Dementia w/ Lewy bodies.
Dosage/Direction for Use
Adult/elderly Initially 5 mg once daily. Dose may be increased to 10 mg/day, following a 1-mth clinical assessment. Max: 10 mg once daily. Patients w/ DLB Dose may be decreased to 5 mg depending on the symptoms.
Administration
May be taken with or without food: Take at bedtime. Place tab on the tongue & allow to disintegrate before swallowing w/ or w/o water.
Contraindications
Hypersensitivity to donepezil or piperidine derivatives.
Special Precautions
Severe Alzheimer's dementia, other types of dementia or other types of memory impairment (eg, age-related cognitive decline). Concomitant use w/ anesth. May have vagotonic effects on heart rate (eg, bradycardia); bladder outflow obstruction. Monitor patients who are at risk for developing ulcers eg, history of ulcer disease or receiving concurrent NSAIDs. Potential to exacerbate or induce extrapyramidal symptoms. Discontinue treatment if NMS occurs. History of asthma or obstructive pulmonary disease. Avoid concomitant use w/ other acetylcholinesterase inhibitors, cholinergic system agonists or antagonists. Severe hepatic impairment. May impair ability to drive or use machines. Pregnancy & lactation. Childn & adolescents <18 yr.
Adverse Reactions
Diarrhoea, muscle cramps, fatigue, nausea, vomiting, headache, insomnia. Common cold; anorexia, hallucinations, agitation, aggressive behaviour, abnormal dreams & nightmares; syncope, dizziness; abdominal disturbance; rash, pruritis; urinary incontinence; pain; accident.
Drug Interactions
Metabolism inhibited by CYP3A4 inhibitors eg, ketoconazole, itraconazole & erythromycin, & CYP2D6 inhibitors eg, quinidine fluoxetine. Increased mean conc w/ ketoconazole. Reduced levels w/ rifampicin, phenytoin, carbamazepine & alcohol. May interfere w/ anticholinergic medications. Potential synergism w/ succinylcholine, other neuromuscular blockers or cholinergic agonists or β-blocking agents.
